I confirm that I am of the full age of majority or, alternately, I am the guardian of the minor participant named, and agree that this Waiver will be binding upon me (as participant or guardian), my heirs, next of kin, executors, administrators and personal representatives.

Sportsmanship & Code of Conduct

I will encourage my child and other team members to have fun. I will behave as a positive role model, respect the goals of the Club, and reinforce the character values of good sportsmanship, teamwork and self-discipline.

The Codes of Conduct establish a clear set of guidelines for coaches, players, parents, guardians and spectators. The primary goal is to ensure that soccer is a fun, positive and enjoyable experience for all involved. The Codes emphasize being a positive role model, respect for the goals of the Club, and reinforce the character values of good sportsmanship, teamwork and self-discipline.

Photo Release

I understand that my son/daughter’s playing as a member of JDF Soccer Association may result in photographs or videos being taken. I consent to the use of my son/daughter’s name, photograph or video while participating as a player with JDF Soccer. By registering at JDF Soccer Association, I give permission for any photographs taken to be posted on websites, social media, print media or television.

Any concerns about this consent should be brought to the attention of the JDF Soccer Association Board of Directors.
